Ohio Cities, Screen 54, and under "City Information" on the left, click on the city that you wish to amend (or add it if it is not there), then at the bottom of the first section (Ohio Cities section) is a box to check and the reason for amending.

You may also click on the RITA form display where it says Amended Return and it will take you right to the place with a yellow highlight.
